2/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Khanilya upon arrival to the home setting. Upon arrival to the home setting, Khanilya appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. Khanilya discussed with QMHP-C her day was fine. QMHP-C engaged Khanilya and Ms. Spencer in a discussion to determine any recent behaviors since the last session. Ms. Spencer indicated Khanilya continues to present problems with anger. 2. Behavior: Khanilya participated in the session without incident. Khanilya appeared to be sad as she was reporting to QMHP-C on how they were picking on her clothes and shoes in the school setting which made her cry and be emotionally out of control. 1. Intervention: QMHP-C began session by inquiring about Khanilyaâs current thoughts, feelings and emotions. QMHP-C allotted time for Khanilya to write down feelings and thoughts that has been occurring since last session. QMHP-C processed with Khanilya on those feelings. QMHP-C assisted Khanilya with contributing factors that causes her depressive feelings and thoughts and learning how to deal with those feelings when they happen. 2. Intervention: QMHP-C engaged Khanilya in writing for instances jotting down a couple of feelings, contacting QMHP-C, and engaging in positive self-talk. QMHP-C provided Khanilya several ways positive self-talking can help her get through difficult moments. QMHP-C role-played with Khanilya different techniques to demonstrate appropriate ways to show feelings. QMHP-C openly discussed the importance of expressing feelings of depressive moods and not holding in feelings inside as she then will explode in a negative way when time presents itself. 1. Response: Khanilya appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Khanilya was able to identify feelings and thoughts since the last session as gave QMHP-C examples of situations that took place in the home that caused her moods to change. 2. Response: Khanilya shared that she likes to verbally express and share rather than write her feelings. Khanilya continues to report that it's difficult to communicate with Ms. Spencer on issues for they are mainly issues about these females that are causing her trouble while she is at home.
